The Consent Form
Parents/adults with legal parental responsibility have a fundamental, legal and ethical right to determine what happens to their infant's body. Valid consent to treatment is therefore absolutely central in all forms of healthcare, from providing personal care to undertaking major surgery.
D-Restricted Ltd will discuss with you in depth your consent form during the consultation. You are entitled to receive a copy of this-should you wish to have a copy please directly request it during the appointment.
This should be considered in-line with the risks that are discussed in detail HERE, and also in person at your consultation.
