Service Description

This appointment is for a reassessment or re-division of a (suspected) restrictive frenulum where I have provided the original and first division. If an alternative practitioner has provided the first division please call me in the first instance to discuss your case before booking this as ideally the original practitioner is best placed to assess the clinical need and has the original notes to hand. My contact number is 07910608179. A reassessment may be suggested to you by myself as part of my follow-up communications with you. Should this be the case I would not require you to complete an additional 'Health Questionnaire'. If this is our first consultation together then please also kindly complete the health questionnaire, located in the tabs on the website page or at www.tongue-tie.info/health-questionnaire. Cancellation Policy

D-Restricted Ltd:  Cancellation & Refund Policy Sometimes, unavoidable things happen which may mean that a client would need to cancel or reschedule their appointment, and here at D-Restricted Ltd, I understand this.  Should a service-user wish to cancel their appointment at any point, they are encouraged to do so at their earliest convenience. D-Restricted Ltd usually have more than one last minute emergency referral that commonly need to make an urgent appointment; who may appreciate an earlier time slot should one become available. Unfortunately, D-Restricted Ltd are unable to refund your booking fee, unless plans are in place to reschedule the original consultation.  In this instance the booking fee is not lost, and is transferred to apply to the deferred consultation.  In the event that less than 4 hours notice is given, the deposit and full fee is lost.  If a service user is running late for their appointment, service users are encouraged to call D-Restricted Ltd (07910608179) to discuss their options.  If the estimated time of arrival is in excess of 10 minutes later than the original scheduled time of the appointment, the consultation will need too be cancelled and the full fee is lost. Should D-Restricted Ltd need to cancel, D-Restricted Ltd will do so as early as possible and do our very best to rearrange the appointment with minimal delay, or refund your full fee so a service user has the option to choose an alternative provider if necessary (www.tongue-tie.org.uk/find-a-practitioner). In the event of a sudden serious illness or critical accident where D-Restricted Ltd would be unable to contact you, or in the event of my death, all appointments would be cancelled by a third party (also healthcare professional who is DBS checked) and arrange a full refund as able. Of course, the nominated person will need to gain access to your name, telephone number and payment details in order to do so and access to this is protected so may take a while for them to access this information legally.  The Association of Tongue-Tie Practitioners (ATP) would be made aware to remove my name from their approved listings, and notices would be put on both my website and social media pages.​ Patient records will still be available should you wish to view them for 25 years post division. If in the event D-Restricted Ltd has deceased, please contact Information Commissioner’s Office (ICO) (details below) for details of any successor who will be able to arrange this for you ensuring you can provide the required formal identification.
